It happens to me and it happens to my opponents. Just now I positioned someone for a backstab, the animation started, and just as I was about to stab, I was suddenly petrified, and my opponent did the backstab animation from in front of me and I fell to the ground but took no damage.

I was always wondering how I couldn't ever get the animation to occur. Apparently you have to manually lower what's in the left hand before attacking. Don't block, and better yet don't hold any other buttons at the time you try to backstab. Also to riposte you have to parry first, this is done with the L2 button and a shield (you swing the shield a bit).
